The role of the family doctor in the care of patients with anxiety-depressive disorders

Abstract
Introduction.Urbanization with an accelerating pace of life, tension in the professional sphere, the need for daily travel at least in the “home–work–home” mode, which often requires a significant amount of time, lack of rest in general and sleep in particular, the growing information boom, significant crowding of people at home, in transport and, often, in the workplace are expected sources of stress. At the same time, additional sources of stress have emerged, such as the COVID-19 pandemic and its consequences, the tense socio-economic situation in the world, etc. All these factors negatively affect human mental health, cause excessive psycho-emotional stress and, in some cases, lead to the formation of various psychopathological conditions.The purpose of the study.To assess the effectiveness of modern approaches to the management of patients with anxiety and depressive disorders in outpatient settings in order to improve the quality of medical care and clinical outcomes, based on a systematic analysis of current literature.Materials and methods.An analysis of regulatory documents and a review of domestic and foreign publications on the management of patients with anxiety-depressive disorders in primary health care were conducted.Results and discussion.The analysis of literature and clinical cases confirmed the high prevalence of anxiety and depressive disorders and the difficulties of timely diagnosis in outpatient settings due to their masking by somatic symptoms and the stigma associated with psychiatric diagnoses. The most common manifestations of anxiety and depression are described, including both psychological and somatic complaints affecting various body systems. The use of a stepwise screening algorithm with validated scales (PHQ-9, GAD-7, HADS, etc.) and modern digital tools (e.g., the “Neuroscanner” app) is justified. The importance of a comprehensive approach involving pharmacotherapy, psychotherapy, and patient education is emphasized to improve treatment adherence and clinical outcomes.Conclusion.The family physician plays a key role in the early detection and management of patients with anxiety and depressive disorders, particularly at the level of primary health care. The use of stepwise screening and modern diagnostic tools helps improve the quality of care and prevent the chronification of psychoemotional disorders.
