Accentuating temperament features in patients with axial spondyloarthritis in accordance with clinical stages

Abstract
Introduction.In recent years, there has been an increased interest in studying the psychological characteristics of patients with spondylarthritis, as this disease disrupts the psychophysiological adaptation of patients, significantly deteriorating their quality of life. The enhanced individual properties of temperament disrupt the intersystem equilibrium forming “inefficient” interaction with reality and constitute a risk factor for psychosomatic disorders. It was assumed that individuals with axial spondyloarthritis (ax Sp A) have accentuated (enhanced) certain temperament traits, which may increase the risk of the disease and affect its course.Our objectivewas to identify accentuated temperament characteristics in patients with axial spondyloarthritis, to establish the dependence of the psychological status of patients on the duration and stage of the disease, and to distinguish between pre-existing temperament traits and those that develop secondary to the disease.Materials and methods.The assessment of temperament accentuations was conducted in 75 patients with axial spondyloarthritis. The first group consisted of 25 patients with axial non-X-ray spondyloarthritis and with active sacroiliitis diagnosed by magnetic resonance imaging with a disease duration of up to 3 years, the second group consisted of 35 patients with axial X-ray spondyloarthritis (stage II sacroiliitis) with a duration of 3 to 5 years, the third group comprised 15 patients with a duration of more than 5–10 years (stage III–IV sacroiliitis). All respondents were tested using the assessment of temperament accentuations questionnaire along with a clinical examination.Results.It was found that in patients with non-X-ray spondyloarthritis, compared with the normative group, the scales of shyness and aggressiveness were significantly reduced (p <0.05); the scale of hyperthymia was increased (p <0.05). With an increase in the duration and radiological stage of axial spondyloarthritis, there is an increase in the deficit of activity in various forms of its manifestation: social and objective at the same time (a decrease in the scales of social activity and hypo-thymity (p <0.05). There was a statistically significant increase in the types of accentuation among patients with stage II and III axial spondyloarthritis, combining features of emotionally unstable and socially and subjectively passive types (p <0.05). A statistically significant (p <0.05) decrease in the aggressiveness scale was revealed in men with HLA-B27 associated axial spondyloarthritis.Conclusion.Information about the types of accentuation of temperament properties and their role in the genesis of spondyloarthritis could expand existing understanding of the risk factors and mechanisms of formation of non-pathogenological SPA in radiological axial spondyloarthritis, possibilities for early pathogenetic treatment and prevention strategies of the disease.
