Ожирение как один из ключевых независимых факторов риска развития и неблагоприятного течения псориатического артрита

Аннотация

На сегодняшний день в мире наблюдается рост заболеваемости псориатическим артритом, в том числе увеличивается количество случаев высокой активности и тяжелого течения данной патологии, что значительно снижает качество жизни пациентов. Не менее важной проблемой является факт высокой распространенности псориатического артрита среди лиц трудоспособного возраста. Согласно данным литературных источников, более чем половина пациентов с установленным диагнозом «псориатический артрит» относится к числу коморбидных. В качестве одного из наиболее часто встречающихся коморбидных состояний, усугубляющих хронический воспалительный процесс при псориатическом артрите, рассматривается ожирение. Кроме того, современные данные позволяют рассматривать высокий индекс массы тела как ключевой предиктор развития псориатического артрита. Избыточная масса тела не только оказывает дополнительную механическую нагрузку на опорно-двигательный аппарат (в частности, на суставы), но и способствует поддержанию и усугублению хронического воспалительного процесса в организме посредством активации ряда иммунных клеток и стимуляции экспрессии таких наиболее важных провоспалительных цитокинов, как фактор некроза опухолей альфа (ФНОα) и интерлейкинов (ИЛ-23, ИЛ-17), способствующих структурному повреждению суставов, а также снижающих эффективность таргетной терапии. В настоящее время проведено не так много исследований, доказывающих высокую значимость ожирения как основного предиктора, усугубляющего тяжесть течения псориатического артрита, однако актуальность данной проблемы лишь возрастает ввиду роста заболеваемости псориатическим артритом и высокой распространенности ожирения среди населения. Все вышеперечисленное подтверждает необходимость особого внимания к вопросу роли высокого индекса массы тела у лиц, страдающих псориатическим артритом.
